[code-reflection] Integrated: Refactor/Simpliy Thread Ops and Phases

Used the ThreadOp names front to back.  So  Ops named using regex  HAT_[GBL][IS][XYZ]  as they will be in generated code.

Simplified phase transform. All Op's now created in a single simple(er) transform
