XStartOnFirstThread - Possible Patch

Hi,

Here is one way of adding -XstartOnFirstThread for OS X.  I copied the logic
from other parts of the launcher and not too sure if all of the code in the
new ContinueInSameThread function is needed.  Please review and comment.
