Web-Form of JCK Application

as I understand the FAQ we need to fill out an applicaton Web-Form
to get green light from Sun for using the JCK

Since there are no links or websites I can find with that specific
application form, I want to know if there's a different way. For example
a "snailmail" address and the application form as a PDF.
