Thanks Mandy!  There were arguments for and against these locations, and 
some alternate suggestions.  The way the sources are, they can be 
moved.  It may be a build system change to get the include directives 
correct for the _md.h files.
I think the discussion should carry forward in the bug report so that 
the resulting code change matches what people have decided.
