RFR: 8289688: jfr command hangs when it processes invalid file [v3]

Yasumasa Suenaga has updated the pull request incrementally with two additional commits since the last revision:

 - throw IOE when the chunk is finished
 - Revert "Add loop limit to ChunkHeader::refresh"
