Jonathan Gibbons has updated the pull request incrementally with four additional commits since the last revision:
 - MarkdownTransformer: tweak doc comments
 - MarkdownTransformer: change `Lower.replaceIter` to be `private final`
 - MarkdownTransformer: use suggested text for using streams
 - remove obsolete debug code
