[8u40] Request for approval for 806433: parameter_index for type annotation not updated after outer.this added

A significant portion of the review took place on the Crucible system;
however, it focused on the new testing framework (ClassfileInspector.java).

The patch applied cleanly, except for Constructor.java, as langtools SQE
staff added more test cases to that file in 9, and these changes have
not been backported.
