test failures

My recent fix should bring the failure count down to three. I've
updated the wiki.

I also added the following new page:

I'm sure I've missed a few open discussions that haven't been
completely settled or haven't been implemented.
Please expand with anything that you remember to be open and resolve
the issues with your name.
