I am able to reproduce this problem using the “Hello world” applet at http://www.segal.org/Hello/. The applet will run immediately if the page is refreshed, but the problem occurs again unless “Do not show this again for this app” was selected.

I'd suggest filing a bug at <http://bugreporter.apple.com>, provide a link to an applet which reproduces this with the latest version of the Oracle Java 7 applet plug-in, and clearly state that it's a Safari only problem so that the Safari team can investigate it.
